The Prevention First membership club is not considered insurance but is a payment arrangement provided by Alpine Pediatric Dentistry for dental services rendered. This membership is available exclusively to patients of our practice who do not have dental insurance and cannot be considered as pre-payment for any future services or as payment for access to any discounted services. This is for the purpose of providing our patient members who elect to make regular monthly subscription payments towards preventive care instead of paying in full at the time of each dental service.
Orthodontic, restorative or cosmetic services and treatment payments are due at the time that services are rendered. A patient’s membership dues need to be current in order to receive any dental services that are provided through the Prevention First membership club. If there is a lapse of 28 days in payment dues that will require a renewed reactivation fee of six months’ dues.
Our Prevention First membership club discounts cannot be combined with other offers or office discounts. It is the patient’s responsibility to schedule their appointments provided by the membership club. There will be no refunds provided for any dues paid which include failure to schedule and keep appointments. The member’s monthly agreement is with the legal entity of Alpine Pediatric Dentistry. Monthly dues and fees for dental services are subject to change at any time. The payment management services are provided by a third party administrator.
